We're sorry you are having trouble entering your Store and ID#. Here are some tips to help you enter the survey:

 
     
     
 
  • The store number can be found either towards the bottom left of your receipt or beneath the store phone number in the middle of the receipt. It should be preceded by "ST#".

  • The ID # can be found on the upper half of the receipt beneath the text that reads "You will need to enter the following online:". It should be a combination of numbers and letters, up to 12 digits in length, and preceded by "ID#”. If you do not have an ID#, or if your receipt is torn or illegible, unfortunately you will not be able to enter the survey.

  • You do not need to enter "ST#" or "ID#" before either of these numbers in the text boxes.

    Error messages may have to do with the date of your most recent Walmart visit or the way in which you entered your Store # or ID # . We apologize for any inconvenience this may have caused and encourage you to try again.
